LIABILITY WAIVER: READ CAREFULLY BEFORE SIGNING

A copy of this form must be completed, signed and submitted by each Angler and Guide competing in the event. In consideration for the acceptance of my entry, I, for my heirs, executors and administrators, release and forever discharge Tarpon Fly Fishing Invitational, Inc. the members of the tournament committee, Tarpon Lodge, all sponsors, producers, their agents, representatives, successors and assigns of all liabilities, claims, actions, damages, costs, or expenses which I may have against them arising out of or in any way connected with my participation in this event, including travel to and from this event, and including injuries which may be suffered to me before, during or after the event. I understand that this waiver includes any claims based on negligence, action, inaction or interaction of any of the above parties. If we do not receive your signed waiver prior to the tournament, you will not be allowed to participate.

BOAT OWNER/OPERATOR STATEMENT OF LIABILITY COVERAGE AND AGREEMENT OF BOAT OPERATION

A copy of this form must be completed and signed by each Vessel Operator in this event. I hereby assert that the vessel I will operate in this event has liability insurance coverage to include that coverage mandated by the State of Florida and further agree to operate the vessel in accordance with all applicable county, state, and federal regulations.
